It is quick, bleeds little, heals with good cosmesis, and needs no closure. But it \u003cstrong\u003efragments the tissue\u003c\/strong\u003e. A pathologist receiving curettings can often identify the cell type and can almost never read the margin or the depth of invasion.\u003c\/p\u003e\u003cp\u003eA \u003cstrong\u003edermal punch\u003c\/strong\u003e takes a full-thickness cylinder with the architecture intact - epidermis, dermis, and the junction between them in their real relationship. That is what makes depth and margin readable. The cost is a wound that usually needs a suture.\u003c\/p\u003e\u003cp\u003eSo: if the lesion is being \u003cem\u003eremoved\u003c\/em\u003e, a curette is often the better instrument. If it might need \u003cem\u003ediagnosing\u003c\/em\u003e - anything pigmented, anything ulcerated, anything that has changed - the punch is the instrument, and reaching for the curette first can cost the specimen that would have answered the question.\u003c\/p\u003e\u003ch2\u003eSizing a Fox curette\u003c\/h2\u003e\u003cp\u003eThe number on a Fox curette is the \u003cstrong\u003ecup diameter\u003c\/strong\u003e, not the handle or the shaft: 1mm, 2mm, 3mm, 4mm, 5mm and 6mm are all stocked here.\u003c\/p\u003e\u003cp\u003eThe rule is the \u003cstrong\u003esmallest cup that still seats over the lesion\u003c\/strong\u003e. An oversized cup does not scrape more efficiently - it catches surrounding normal skin at the rim, and instead of lifting the lesion cleanly it tears across the boundary. That is the commonest reason a curettage leaves a worse scar than it should have.\u003c\/p\u003e\u003cp\u003ePiffard curettes cover the same ground with a heavier handle and three sizes (3mm, 5mm, 6mm), which suits a firmer scrape on thicker skin.\u003c\/p\u003e\u003ch2\u003eWhy a nail splitter has an anvil\u003c\/h2\u003e\u003cp\u003eCutting a nail plate with ordinary scissors presses the plate \u003cem\u003edown into the nail bed\u003c\/em\u003e as the blades close. On an already inflamed or infected toe that is the painful part of the procedure, and it is avoidable.\u003c\/p\u003e\u003cp\u003eA \u003cstrong\u003enail splitter\u003c\/strong\u003e puts one thin blade underneath the plate and a flat anvil above it. The anvil absorbs the closing force instead of the nail bed, so the plate is divided in place rather than levered. The 2mm Fig. 59 pattern suits a partial nail avulsion; the 13cm anvil pattern is for a full split.\u003c\/p\u003e\u003ch2\u003eComedone extractors\u003c\/h2\u003e\u003cp\u003eSaalfeld, Schamberg, Unna and Walton patterns, in 10cm and 14cm, single- and double-ended.
