Crile or Kelly? The difference nobody explains

These two are the most confused instruments in surgery and they are nearly indistinguishable in a catalogue photograph. The difference is one detail:

Crile jaws are serrated along their full length. Kelly jaws are serrated only on the distal half.

That is it, and it matters. A Kelly's smooth proximal jaw lets it grip a vessel near the tip without the whole jaw biting, which suits a delicate pedicle. A Crile grips along the entire jaw, which holds better on a bulkier bite and is the reason it is the more common general haemostat.

Kocher and Rochester-Ochsner, meanwhile, are the same instrument under two names - a heavy transverse-serrated clamp with 1x2 teeth at the tip. If a tray list calls for one and you stock the other, you already have it.

Toothed or smooth: the decision that goes wrong both ways

A toothed jaw penetrates and holds. A smooth jaw compresses and occludes. Choosing by feel rather than by tissue is how both classic errors happen:

  • A toothed clamp on bowel perforates it - sometimes not visibly, and not until later.
  • A smooth clamp on fascia slips under load, and the repair comes apart where the surgeon believed it was held.

The same error from opposite directions. Allis (toothed) and Babcock (atraumatic, fenestrated) exist as a pair precisely because of this: Allis for tissue being excised, Babcock for tissue staying in the patient.

Why a mosquito is not just a small haemostat

The Halsted mosquito is the finest ratcheted clamp in general use, and its purpose is not simply reaching small vessels. It is working in a field where a full-size haemostat would occlude the view. A 12.5cm mosquito on a 5mm bleeder leaves the surgeon able to see what they are doing; a 16cm Crile on the same bleeder does not.

Stocked in straight, curved, toothed and titanium, in 12.5cm micro patterns.

Ratchet, spring, or neither

Three closing mechanisms, three uses, and they are not interchangeable.

A ratchet holds indefinitely at whatever force the hand applied. Good for a vessel about to be tied. Dangerous on anything where force matters, because under pressure hands set clamps harder than they intend.

A spring - the bulldog patterns - delivers a known, repeatable force calibrated to the jaw. That is why bulldogs and not ratcheted clamps go on a coronary. Jaw length matters here too: a jaw longer than the vessel is wide concentrates the whole spring force at one point.

A tangential clamp occludes part of a vessel's circumference while flow continues past. See Cardiothoracic Instruments for when that is the right choice.

Frequently asked

Is a haemostat the same as a clamp? A haemostat is a clamp used for haemostasis - Crile, Kelly, mosquito, Pean. Aortic, bowel and towel clamps are clamps that are not haemostats. Everything in both groups is here.

Why are some titanium? Titanium is lighter, non-magnetic and does not interfere with imaging. It matters for MRI-adjacent work and for long cases where instrument weight on tissue accumulates.

What length should I order? Length is set by depth of field, not by the size of the vessel. Work at the skin surface takes 12-14cm; a deep pelvic or thoracic field needs 20cm or more, and the Rochester-Pean range runs to 40.5cm for exactly that reason.

Do you stock Kocher and Rochester-Ochsner separately? They are the same pattern; both names appear in the titles because both appear on tray lists.
