Choosing a Blade Length

Blade length is the only dimension that really matters on a nasal speculum, and getting it wrong costs you the view. Too short and the blades do not reach past the vestibule, so the nasal cavity stays closed beyond them. Too long and the blades press on the septum and lateral wall, which is uncomfortable for an awake patient and can cause mucosal trauma.

The graduated sizes exist for exactly this reason: 32mm blades for an adult nose, 30mm for a child, and the Fig. 2/3 infant pattern for the smallest airways. A clinic seeing mixed ages needs at least the adult and child sizes, not one compromise size.

How to Choose

Patient size. Start from the patient, not the procedure. Adult, child, infant and small are graduated blade lengths, and the smallest airway you routinely see decides the smallest speculum you need to stock.

Curette cup size. The 3x5mm cup is the working end that fits into an ethmoid cell. A small shaft keeps the instrument body clear of the nasal opening so you retain a line of sight to the cup.

Elevator. The Halle elevator is used to lift mucoperiosteum off bone — in septal work and in raising flaps. The small pattern at 7 1/4" is the standard intranasal size.

What is a Halle nasal speculum used for?
Opening the nasal vestibule so the nasal cavity can be examined or operated on, and keeping it open while instruments pass alongside. It is used both in clinic examination and in theatre.

What is the difference between Halle and Hartmann-Halle?
Hartmann-Halle is a combination pattern that merges features of the Hartmann and Halle specula. In this catalog the Hartmann-Halle instruments are the ones graduated by patient size; the Halle speculum is listed by blade length.

What is an ethmoid curette used for?
Clearing the ethmoid air cells — removing diseased mucosa, polypoid tissue and bony partitions during ethmoidectomy. The small cup is sized to the cells themselves.

Which sizes should a clinic stock?
At minimum the adult and child sizes. Add the infant pattern if you see neonates or very young children, since an adult speculum cannot be used safely in that airway.
