Kerrisons

The Kerrison punch is the spine surgeon's nibbler. A thin footplate slides under the edge of the lamina or ligament, and squeezing the handle drives a cutting jaw down onto it, taking a precise bite of bone or ligamentum flavum while the footplate protects the dura and nerve roots below.

Why 40º or 90º?

The angle is the angle between the shaft and the cutting jaw. A 90º Kerrison bites straight up, perpendicular to the shaft, which suits bone directly above the footplate. A 40º Kerrison bites forward at an angle, so it reaches under the lamina and along the canal with less rotation of the wrist. Most surgeons keep both angles in each bite size.

How to Choose

Bite size. 1–2mm for the foramen and fine decompression; 3–5mm for lamina and larger bone removal.

Shaft length. 7" for cervical and superficial work; 8–9" for lumbar and deeper fields.

Rotatable or fixed. A rotatable shaft turns the jaw without turning the hand. A fixed Kerrison is simpler and rigid.

Footplate and ejector. A thin footplate slides under tight bone more easily. An ejector pushes the bite out of the jaw so it does not have to be cleared by hand.

Coating. Black coating cuts glare under the microscope.

What is the difference between a Kerrison and a regular rongeur?
A rongeur closes two cupped jaws together. A Kerrison cuts a jaw down onto a footplate, which shields the structures below the bite.

What does upbiting mean?
The jaw cuts upward, away from the footplate. All Kerrisons listed here are upbiting.
