A bulldog clamp has no ratchet. It closes under its own spring tension and it is released by squeezing it open — which is the entire point. On a vessel you intend to repair and reperfuse, a ratcheted clamp applies whatever force the surgeon set; a bulldog applies only the force the spring was built to deliver, every time, on every vessel.

The tissue you are clamping has to work again afterwards. A vessel that has been crushed will thrombose, dissect or tear at the suture line, and the damage is not visible at the time.
A ratcheted clamp is only as gentle as the hand that set it, and under pressure hands set clamps harder than they mean to. A bulldog removes the variable: the spring delivers a known, repeatable closing force, calibrated by the manufacturer to the jaw size. That is why bulldogs, not ratcheted clamps, are what goes on a coronary or a renal artery.
It is also why jaw length matters more than it looks. A clamp whose jaws are longer than the vessel is wide will concentrate its whole spring force at one point instead of distributing it — which is exactly the crush injury you were avoiding.
Jaw length. The jaw should span the vessel's full width and no more. 8–12mm for coronaries and small branch vessels; 2–3cm for mid-calibre peripheral vessels; 4–6cm for larger targets. Order by vessel, not by habit.
Straight or curved jaws. Straight jaws approach a vessel head-on and are easiest to place on a surface vessel. Curved jaws pass around behind a vessel and let the clamp body sit out of the operative axis, so it does not obstruct the suture line you are about to work on.
Angled and cross-action patterns. An angled clamp keeps the handle clear of a crowded field. Cross-action closes when you squeeze rather than when you release, which some surgeons prefer for one-handed placement.
Titanium. Lighter and non-magnetic. The separate artery and vein patterns here differ in spring tension — a vein needs less force to occlude than an artery of the same calibre.
An applier. The Johns Hopkins applier places and removes small bulldogs at depth without your fingers entering the field. Worth having if you use the small sizes.
What is a bulldog clamp used for?
Temporary occlusion of a blood vessel — stopping flow during anastomosis, repair or grafting without crushing the vessel wall. Standard in cardiac, vascular, transplant and microvascular surgery.
Why does a bulldog clamp have no ratchet?
Because the spring supplies a fixed, repeatable closing force. A ratchet would let the surgeon apply more pressure than the vessel can tolerate, and vessel crush injury is not visible at the time it happens.
What is the difference between a bulldog clamp and a vascular clamp?
Scale and mechanism. A bulldog is a small spring clamp for a single vessel. A larger vascular clamp — aortic, Satinsky, DeBakey ring-handle — uses a ratchet and is sized for major vessels and partial occlusion.
Which size should I order?
Match the jaw to the vessel width. Too long and the force concentrates at one point; too short and the vessel is not fully occluded. Most cardiovascular sets carry several sizes rather than one.
