This is the single thing most worth checking before ordering, and the place buyers most often go wrong.
A surgical set is sold as complete, but complete always means complete with a specific complement - a named number of blades, tips, or sizes. Two sets can carry almost the same name and contain materially different things. In this collection alone:
Same frame, same word “complete”, different instrument in use. The same pattern runs through the Kirschner abdominal sets (frame plus four blades, versus the round 340×240mm version), the Caspar counter-retractor set (six blades), and the Henley retractor set (four centre blades and three pairs of side blades).
Read the complement, not the pattern name.
Where a retractor set offers both, the choice is about what the blade is anchoring in, not about how aggressive you want to be.
Sharp blades bite into fascia and muscle and hold position against sustained retraction force. On a deep exposure where the tissue would otherwise creep back over the course of an hour, that anchorage is the whole point of the set.
Blunt blades spread load across a surface instead of anchoring in it. They are for tissue you cannot afford to perforate - vessels, nerve roots, bowel, dura - and for the cases where the retractor sits against a structure rather than in it.
Buying on price between two otherwise identical sets is how a theatre ends up with the wrong blades, because the price difference between sharp and blunt is usually small and the difference in use is not.
A fitted tray reads like an upsell. It is doing two jobs that matter.
It makes the count auditable. A set in a fitted tray has a visible empty slot when a piece is missing. A set in a pouch does not, and the missing piece is discovered at the next case rather than at the end of this one.
It protects the working ends through reprocessing. Instruments that move against each other in a washer and an autoclave arrive with rolled edges and bent tips. The tray is what stops a graduated dilator set becoming a set with two damaged sizes in it.
A set bought without its tray tends to become a loose set within a year - the pieces separate across trays and the set stops functioning as a set.
Both are reasonable. The rule that decides it is mechanical fit.
Buy the set when the pieces have to fit each other: a retractor frame and its blades, a graduated dilator progression, a speculum set sized in steps, a laryngoscope handle and its blades. Here the set is not a discount on buying separately - it is the guarantee that the parts mate, and sourcing a replacement blade for an orphan frame years later is a genuinely difficult job.
Build your own when the instruments merely coexist on a tray - a general tray, a minor procedure tray, a suture set. Nothing has to fit anything, so you can match a surgeon's actual preferences instead of accepting a manufacturer's idea of a standard tray.
Can I buy a replacement blade separately? For many of the retractor systems, yes - blades and frames are listed individually as well as in sets. Search the pattern name to see what is available for that system.
Are the dilator sets graduated or single-size? Graduated. The Hegar sets run 8, 14 and 26 pieces covering sizes 1-26mm, and the figure in the title is the piece count, not the size.
Do the sets come sterile? No - these are reusable instruments supplied non-sterile and processed in your own facility.
What is the difference between a set and a kit here? Nothing consistent. Both terms appear in manufacturers' own names for the same kind of thing, so the collection carries both rather than imposing a distinction that does not exist in the trade.
