The shaft diameter is fixed by the cannula, so the port plan decides the instrument set rather than the other way round. A 5mm shaft passes every 5mm and 10mm port; a 10mm shaft passes only the 10mm. That asymmetry is why most of a lap tray is 5mm and the 10mm instruments are the few that need the diameter to work - large clip appliers, specimen graspers, and the 10mm Maryland dissector where the jaw has to be substantial enough to hold a pedicle.
Length matters as much as diameter and gets less attention. A 33cm shaft is standard; 45cm exists for bariatric and high-BMI cases where the port-to-target distance defeats a 33cm instrument. Buying a 33cm-only set and then meeting a 45cm case mid-list is a common and avoidable problem.
A one-piece laparoscopic instrument is simpler and cheaper to buy. When its jaw wears or its insulation fails, the whole instrument is scrap.
A modular instrument separates into insert, outer sheath and handle. The insert is the part that wears - it takes the tissue load and the cleaning cycles - and on a modular system it is the only part replaced. Over a five-year service life the modular route usually costs less per case despite the higher purchase price, and it makes cleaning verification easier because the lumen opens up. The trade-off is assembly discipline: a modular instrument assembled wrong is a modular instrument that fails in the middle of a case.
A monopolar laparoscopic instrument carries current down a long, thin, repeatedly autoclaved shaft. The insulation on that shaft develops pinholes - from handling, from cleaning brushes, from thermal cycling - and a pinhole is invisible to the naked eye.
What makes it serious is geometry. The visible field of a laparoscope is a small cone; the shaft passes through tissue well outside it. Current leaving through a pinhole 10cm behind the jaw produces a burn nobody sees at the time, in bowel or vessel, and presents days later. This is the reason insulation testing exists as a discrete step in instrument processing rather than an optional extra, and why an insulation tester belongs alongside a monopolar lap set rather than being treated as a nice-to-have.
Bipolar instruments contain the current path between their own two jaws and remove this failure mode entirely, which is part of why they cost more.
These three cover most of what a lap tray grasps, and they are not interchangeable.
The Maryland has a fine, curved, tapered jaw. It is a dissector first and a grasper second - the curve lets it pass behind a structure and open to create a plane. Use it to develop tissue, not to hold it under load.
The Babcock has a broad, fenestrated, atraumatic jaw shaped to hold bowel without crushing it. Wide surface, low pressure. It holds; it does not dissect.
The crocodile has multiple interdigitating teeth along the jaw. It grips hard and it marks whatever it grips - right for a specimen, a suture, or a piece of mesh, wrong for anything staying in the patient.
The general rule holds here as elsewhere: the atraumatic jaw that slips on fascia and the toothed jaw that perforates bowel are the same error from opposite directions.
Are these reusable or single-use? Reusable, autoclavable instruments. The disposable items in the catalog sit under endoscopy disposables rather than here.
Can I order a modular insert on its own? Yes - inserts, sheaths and handles are listed separately where the system is modular, so a worn insert is replaced without buying a complete instrument.
Do the electrosurgical instruments include a cable? Some are listed with cable and some without; the product title states which. Check before ordering, because cable fittings are not universal across generators.
What is the difference between the 40°, 70° and 90° articulating instruments? The figure is the maximum tip deflection. Higher articulation reaches around a structure from a straight port line, at the cost of some rigidity at full deflection.
